Why can a patient with terminal illness refuse treatment, while, we have to go through forced treatment.

The system is messed up. Has anyone ever sucessfully appealed?

Definitely. In New Zealand there is very little they can do to make you. You just don't tell them you're still like that. And they let you go to CTB another day. Kinda sad really

Double check the law in your state/municipality. This is the one determinant of what is most likely going to happen.

I brushed up on the legal obligations for my therapist and my family doctor before i went in to see them, and asked them upfront too. They are very restricted by these rules. It was quite easy to walk around and not set off any unnecessary alarms, or come to a point where i would have to be committed to a psych ward (which they can do involuntarily).

In my part of the world, the most they can do is call the police for a welfare check. If you aren't too unstable and you're amicable/articulate/sober, these are easy to get through. You juts have to convince a cop youre not an imminent risk to yourself or anyone else.
