What kind of discord were you in? If you go around telling random people that you're planning to ctb and give the specifics on how you plan to do it, you've invited them into your personal business and they don't want to feel responsible for standing by and allowing you to die. Making public statements like this can be interpreted as a "cry for help" for which they answered in the only way they knew how. Your discord friends probably reported you to the admins and after some internal sleuthing to narrow down your identity they (discord) contacted the authorities.
Discord as a communications platform that does get used by bad actors to plan and commit crimes absolutely has ways to identify and track down users and pass that info to LE to a reasonable extent. People have been arrested for posts on much more anonymous sites than discord.

They're not going to tell you the details of their investigation. If you handed them SN, they would have had you commited. You'd still be an immediate suicide risk.

About an hour ago the police came to my front door telling me Discord has given them information I had bought sodium nitrite with the intent to commit suicide. They just asked for the sodium nitrite, I gave it to them and they went their marry way. Just a quick warning, a mere slap on the wrist.

I think they meant that the general operation (of delaying inevitable death*), began in November... but the person-specific part of the operation could have began much later.

They implied they were using data from that account, so this was a months long operation. Discord deletes account data after 30 days of being banned...

I think Discord *claims* to delete data, no guarantees, just 'trust me'. Trust without proof of non-betrayal. Makes me wonder what other services will betray the users who trust them.
They can easily find you via IP. If you use Discord, you give them your IP. This IP is handed out to any law enforcement organization and from there it's trivial to find your ISP and from there, find out who and where you are. I'd be surprised if the entire process isn't automated tbh. You'd think that these various corporations would not give your data out willy-nilly but in reality they don't give a shit and are woried they'll get sanctioned by governments if they don't cooperate blindly. Which yes, governments did in the past to companies with concience.
Next time police come without a warrent you can shut the door in their face and tell them to fuck off unless they have a Warrent singed by a judge. You have that right as a US citizen.
Actually the case in many EU countries too. Police is specifically trained and exprienced in making you think they have rights they haven't. When I read here that police comes to people's homes and e.g. search them for things like SN with no court order... well I can't say I'm shocked because that's how the government thugs operate, but you don't have to cooperate.
Don't talk to the police. Don't answer questions. Only give them your ID when asked. Repeat that you would like them to leave. Stay friendly, but persistent. If they come in anyways, don't sign anything. Don't agree. State clearly (and best, with a witness) that you have not agreed to a search.
I understand it's easier said than done, especially since they're also trained in intimidating you. But it's the proper way to do it.
I have never understood why people think it is a good idea to use discord if they care about their privacy.
Not judging you OP, you gotta do what you gotta do and it sounds like you're just an average joe like me who isn't up on cutting edge IT stuff.
What I find odd however, is that there are people who are ostensibly concerned about privacy who use and recommend discord even though it seems to be a privacy concern.
There's gotta be a better way to do whatever they do and if there isn't it would at least be worth mentioning to people that discord is not the greatest when it comes to privacy but most people act like it is private.
