My plan is to throw my rope over a door and shut it. But what if during my convulsions i open the door or something? How do i avoid risking that?

You should lock the door with keys or a latch for more security.

With the second variant, you won't run the risk of falling if the door opens, since the body and the object that acts as an "anchor" are on the same side.

The first link contains a photo of how everything would look assembled. The second link contains a video of an actual attempt (where the door opens and the man does not fall).

Anything that serves as an 'anchor' and doesn't come apart or pass/slip through to the other side will be fine.

A simple stopper knot might work but I personally would prefer to tie something else to the other side of the door.
