I have an appointment in about four hours. My medication hasn't been working lately, but I'm at the maximum dosage for my BMI. I'm feeling pretty bad and I've been pretty suicidal. I want to talk about ways to deal with this without risking going back to the psych ward. What phrases should I use/avoid? I want to be as honest as possible.

Just say you're having suicidal thoughts. DO NOT mention anything about acting out on those thoughts, they will test you to see if you want to actually do it. They will ask "do you have any plans to suicide?" Say no. Do say yes when they ask you if you have thoughts. The best they will do is give you medication that treats suicidal thoughts.

No am in England, it should be somewhat similar in the U.S

It depends on the state. Here in Florida, where the Baker Act is in effect, the mere remote mention of suicidiality will get you put in the psych ward.
